b.具有良好的商業信譽和健全的財務會計制度：具體要求：提供本企業2020年度或2021年度第三方會計師事務所出具的財務審計報告或開戶銀行出具的資信證明；

c.具有履行合同所必需的設備和專業技術能力：具體要求：提供具備履行合同所必需的設備和專業技術能力的證明材料或書面聲明（格式自擬）；

d.具有依法繳納稅收和社會保障資金的良好記錄：

具體要求：提供2021年以來任意三個月依法繳納稅收和社會保障資金的有效證明材料（復印件加蓋公章）；

e.參加本次采購活動前三年內，在經營活動中沒有違法違規記錄：

提供參加采購活動前3年內在經營活動中沒有重大違法記錄的書面聲明（格式文件詳見投標文件范本）；

f.法律、行政法規規定的其他條件：

供應商須承諾：在“信用中國”網站 、中國 等渠道查詢中未被列入失信被執行人名單、重大稅收違法案件當事人名單、政府采購嚴重違法失信行為記錄名單中，如被列入失信被執行人、重大稅收違法案件當事人名單、政府采購嚴重違法失信行為記錄名單中的供應商取消其投標資格，并承擔由此造成的一切法律責任及后果。
