normal style='text-indent:28.0pt;line-height:150%'>1.必須符合《政府采購法》第二十二條規定；

normal style='text-indent:28.0pt;line-height:150%'>（1）  具有獨立承擔民事責任的能力：提供法人或其他組織的營業執照等證明文件（營業照副本、組織機構代碼證副本和稅務登記證副本，或三證合一的營業執照）或自然人身份證 ；

normal style='text-indent:28.0pt;line-height:150%'>（2） 法定代表人參加投標的必須有法定代表人身份證，法定代表人授權委托人參加投標的必須有法定代表人授權委托書及被授權委托人身份證；

normal style='text-indent:28.0pt;line-height:150%'>（3）具有良好的商業信譽和健全的財務會計制度：

normal style='text-indent:28.0pt;line-height:150%'>具體要求：提供2020年度或2021年財務審計報告（包含資產負債表、利潤表、現金流量表及報表附注，注：審計報告要加蓋審計機構公章，要提供審計機構的營業執照和執業資格證）或基本開戶行的銀行2021年至今出具的有效資信證明（至出具之日起2個月有效）或財政部門認可的政府采購專業擔保機構出具的有效擔保函，并承諾有良好的商業信用和健全的財務制度；

normal style='text-indent:28.0pt;line-height:150%'>（5）具有依法繳納稅收和社保繳納證明的良好記錄：

normal style='text-indent:28.0pt;line-height:150%'>具體要求：提供依法繳納稅收（2021年以來任意連續3個月）和社保繳納證明（2021年以來任意連續3個月）的相關證明材料。未發生繳稅情況的，須提供零申報證明，即提供企業所在地稅務部門出具的申報證明或加蓋稅務機關鮮章的申報表，從網上系統打印的需加蓋投標人鮮章；提供2021年以來任意連續3個月發生并繳納社保的有效證明材料；

normal style='text-indent:28.0pt;line-height:150%'>（6）參加本次政府采購活動前三年內，在經營活動中沒有重大違法記錄的聲明：提供參加政府采購活動前3年內本公司及法定代表人在經營活動中沒有重大違法記錄的書面聲明；

normal style='text-indent:28.0pt;line-height:150%'>（7） 誠信資格要求：對列入失信被執行人、重大稅收違法案件當事人名單、政府采購嚴重違法失信行為記錄名單的供應商，拒絕其參與報價。查詢截圖加蓋鮮章,提供在“信用中國”網站（包括信用信息首頁(法人和其他組織信用信息概述)，失信被執行人、重大稅收違法案件當事人名單、政府采購嚴重違法失信名單）、中國 （包括政府采購嚴重違法失信行為記錄名單）查詢時間為公告之日至開標截止時間前任意時間；（提供自本項目采購公告發布之日起至投標截止時間止任一時間段的檢索查詢結果，）提供帶網址及版權信息的查詢頁面完整的彩色查詢記錄截圖（完整清晰）；

normal style='text-indent:28.0pt;line-height:150%'>（8）提供中國裁判文書網無行賄、受賄犯罪證明，無行賄受賄犯罪記錄查驗證：投標供應商須在中國裁判文書網、檢索查詢企業以及企業法定代表人是否存在行賄、受賄等犯罪檔案記錄，若自本項目采購公告發布之日起至投標截止時間止前3年內存在行賄、受賄等犯罪檔案記錄的將取消其投標資格，提供帶網址及版權信息的查詢頁面完整的彩色截圖 （完整清晰）；
